The Resident Wellness Director (Director of Nursing) is responsible for the overall management of the health and wellness of the residents.
Responsibilities include but are not limited to overall well-being of the residents, training team members for the home; team member recognition, annual performance reviews, and ongoing coaching and counseling of team members; when necessary facilitation of resident care conferences and the completion of Service Plans; completing assurance programs as per policy.
It is expected that the Resident Wellness Director complete any required training and become knowledgeable in the Company philosophy, principles of service, and policies and procedures.
This knowledge becomes the foundation for future decision making.
As the leader of the general assisted living community, the Resident Wellness Director is expected to develop and maintain a positive atmosphere for residents, families, visitors, and team members.
Resident Wellness Director:
Ensure each resident has a current and updated Service Plan according to policy and state regulatory guidelines.
Complete state specific assessments/clinical tools as appropriate.
Attend Service Plan meetings Ensure that Company philosophy and principles are implemented on an ongoing basis.
Monitor the 24-Hour Report for entries indicating physical/behavioral changes and proper/complete documentation.
Communicate regularly with families and physician as necessary.
Work with Director of Sales to schedule assessments.
Initial Resident Assessments.
Wellness Visits.
Discuss service level changes to include Incontinence Program and Med Fees with Executive Director.
Identify the on-going needs of all residents in order to maintain their health and wellness.
Illness visits.
Be available by phone for questions regarding the medical conditions of the residents.
Act as a liaison of all health services such as Home Health or Mental Health Associations, etc.
, to facilitate communication and ensure that all necessary services are being provided.
Act as a liaison with the pharmacy to ensure effective services to residents, families and team members and to ensure regulatory compliance.
Maintain communication and assist with the coordination of services with other health care facilities to ensure the continuum of care for residents who have been transferred to other health care facilities.
Maintain inventory of personal, medical and incontinence supplies to include first aid kits for community.
Coordinate the incontinence Program.
Provide emergency first aid to residents and team members as needed.
Monitor all infections, and/or accidents/incidents of residents.
Monitor compliance of all infection control policies to include infection control rounds.
Provide consultation and technical assistance to team members as needed.
Maintain resident confidentiality and coach others on its importance.
Maintain Wellness Files and ensure proper documentation of all changes of condition and health care services for each resident.
Assist with diet board/book.
Training:
Participate in the ongoing structured orientation program for all new team members.
Ensure all new team members complete orientation in compliance with Company policy and state regulatory guidelines.
Provide regular, ongoing training as necessary to team members.
Medication Aid Supervision:
Assist with the implementation of all required training.
Self-Administration Medication Management Supervision.
Regularly monitor each resident's medication schedule.
Oversee the documentation completed for each resident.
Assist Medication Aids with documentation skills to ensure that all records are properly maintained.
Facilitate Communication:
Assist in counseling residents and family members during transitional periods.
Regularly contact hospitals or other facilities in which residents are temporarily residing to obtain updates on their condition.
Call family members of residents who are in the hospital at least twice (2) per week to provide support.
Communicate pharmacy issues.
Regulatory Issues:
Assist with/Coordinate provision of state required training programs with Resident Care Manager and Executive Director.
Assist with compliance of OSHA requirements for all team members.
Develop a working knowledge of state regulations, Inspirit Senior Living policies and procedures dictated for residents and ensure compliance.
Quality Assurance:
Review all resident and/or health services Incident Reports and develop a plan of correction to increase safety for both team members and residents.
Attend meetings as requested including QA improvement meetings and Nursing meetings.
Review pharmacy consultant reports and develop plan of correction as necessary.
Review the accuracy of all physician orders monthly.
Administrative:
Complete team member performance reviews on a timely basis, both provisional reviews and annual reviews.
Ensure tickler system for maintaining is in place.
Leadership:
Provide daily coaching and support to team members in the community.
Lead by example, being visible, involved, and accessible.
Facilitate/attend both Stand-Up meetings and Cross-Over meetings.
Preferred
Qualifications:
Education:
Maintain a current state license as a professional nurse.
Experience:
Two (2) years long-term care experience in personal care or healthcare management.
Licensure:
Current LPN.
Testing:
Pre-placement drug test and criminal background check.
Knowledge/Skills/Abilities:
Good organizational and time management skills.
Exhibits a caring and compassionate attitude while articulating true concern for people.
Able to work flexible hours and be available to respond during off duty hours to community problems and emergencies.
Possess good judgement, problem solving, and decision-making skills.
Possess the ability to effectively read, write and communicate in the language of the residents.
